DOH Seal Developmental Disabilities Supports Division Promising Practices

2010 Director's Community Inclusion Awards
The Developmental Disabilities Supports Division seeks nominations for the 2010 Director’s Community Inclusion Awards for July 2009 – June 2010. Award nominations has been extended to Wednesday December 8, 2010 and must be received by mail or fax by the Regional Director (attach a copy of this page to your submission).

Anyone may submit an Award Nomination Success Story about adult New Mexicans with developmental disabilities using the forms (in a separate document) specific to each category listed above. A signed authorization form to photograph, interview, write and publish images and stories (see attachment) must also be attached to each award nomination form.

The award categories are Self-Employment, Entered Community Employment from Adult Habilitation, Community Employer/Employee Partnership, Community Based Adult Habilitation Services, Individual Community Membership and Full Community Inclusion and Employment Champion. Self-Employment
Use this form to nominate:

  • Business Owner of the Year: a person with developmental disabilities who achieved successful self-employment (i.e., started his/her self-owned business)
  • Outstanding Employee: Business Owner of the Year: the individual who most supported this person to self-employment (the 2008 winner was a direct support staff member)

Entered Community Employment From Adult Habilitation

  • Community Employment, Employee Success of the Year
  • Successful Transition to Community Employment from Adult Habilitation Services: Team Support to Success
