Developmental Disabilities Supports Division (DDSD) Requirements - Procedures:

DDSD Procedures:

Centralized Admission and Discharge Process for New Mexicans with Developmental Disabilities in pdf adobemark- The purpose is to respond promptly to the needs of a person with a developmental disability whose immediate health and/or safety would be jeopardized if not provided intervention.

Expedited Allocation to the Developmental Disability Waiver Program in pdfadobemark– June 18, 2007 – The purpose of this procedure is to establish a process by which an individual on the Central Registry who meets the federal definition can be allocated to Waiver services on a basis other than the individual’s date of registration.


Medication Administration Assessment Proceedure in pdfadobemark - This procedure for individuals on the DD Waiver and DD Adult State General Funded services ensure the health and safety of adults in Community Living Services, Adult Habilitation or Employment services. The Medication Assessment and Delivery policy, procedure and Medication Administration Assessment Tool (MAAT) clarify the level and type of assistance needed with the delivery of medications.

Medication Administration Assessment Tool (MAAT)word.doc in MS Word Form- This is the form to be used by New Mexico nurses for assessing the level and type of assistance needed with medication delivery.


Referral to DVR for Employment-Related Services in pdfadobemark - Eligible individuals shall be referred by DD Waiver providers to DVR under these referral conditions.

State General Fund (SGF) and Family Infant Toddler (FIT) Billing Procedure in pdfadobemark - This procedure formalizes the process and provides the billing form for completion and submission of all invoices submitted directly to DDSD by Contractors and Providers.  By ensuring that providers following this procedure, DDSD hopes to obtain timely payment of all complete and correct invoices.


Supporting People on the DD Waiver with Dysphagia/Risk for Aspiration in pdf adobemark- The purpose is to clarify roles for all therapists/eating specialists, case managers, agency nurses and nutritionists state-wide serving individuals on DD waiver who have been identified as having Dysphagia/Risk for Aspiration.

DDSD Request for Regional Office Intervention form - Anyone may complete and submit a request to their local regional office.  Before a Regional Intervention Form is submitted, however, the person or provider planning to do so must try all avenues available to them for resolving the problem.  This information must be on the Regional Intervention Form when submitted so the regional office staff knows what has already been done and where to start assisting.  The Regional Intervention Form is also used to report specialty service needs that cannot be met through regular channels or within appropriate timelines.

Vocational Assessment Profile Procedure for Individuals on the Developmental Disabilities Waiver Who Are and Who Are Not Jackson Class Members in pdfadobemark - This procedure outlines how individuals, groups and teams will use Vocational Assessment Profiles (VAPs) to support each DDW participant to get the career he/she wants.  Applicable individuals, groups and teams include, but are not limited to: DDSD staff, DDSD Case Managers, DDW Providers, DDSD approved VAP Facilitators, DDSD approved VAP Mentors, DDSD Community Inclusion Supported Employment Coordinators and other Interdisciplinary Team (IDT) members.
