Individual Service Plan (ISP)
The Department of Health (DOH) regulations governing the Individual Service Plan (ISP) for people served by the DD Waiver have been revised effective 10/01/06. As a result, the ISP form and instructions have also been revised effective 10/01/06. These changes are the result of work from a longstanding workgroup and pilot project that occurred in 2005. The new ISP form and process begin with the individual’s history, life experiences, preferences and vision for his/her future. This information will clearly lead the individual and his/her team into developing outcomes in various life areas that will assist the individual in fulfilling his/her dreams through outcomes that can easily be measured. The ISP form can also be completed electronically by case managers and other providers.

What’s New about the ISP?
The ISP forms are available as documents in MS Word that have fields that can be filled out directly into the document.
