The Developmental Disabilities Supports Division (DDSD) Request for Proposals (RFP):

Services and Projects Statewide to Serve Children & Adults with Developmental Disabilities and Their Families

The purpose of this procurement is to provide services and supports to individuals with, or at risk for, developmental disabilities and their families. Individuals and their families have asked for the services and supports identified within this RFP. Strategies specific to this Performance Measure are: Provide information and support to individuals, parents, and guardians that supports timely access to services and promotes self advocacy and self determination..

This RFP will assist the Department to meet portions of its 2011 Strategic Plan that relate to the Department’s mission to prevent, protect, provide, promote and partner to improve health services systems and assure that critical public health functions and safety net services are available. Further, a specific measure identified is: Promote independence and quality of life for individuals with developmental disabilities.   

In order to ensure that the DOH has the benefit of the highest quality and most cost effective services available, DDSD will assure that selection of the successful Offerors is based on open and free competition in accordance with State law. Agencies including public, private, government, for profit/non-profit and individuals with relevant experience to the population(s) and/or project(s) identified in this RFP are encouraged to submit proposals. The DDSD has established guidelines for all contracts:

1. Offerors must have, or be willing to obtain, a current New Mexico tax identification number from the New Mexico Taxation and Revenue Department.

2. Offerors are encouraged to detail efforts in collaboration with Temporary Assistance of Needy Families (TANF) and New Mexico Welfare to Work Offerors, to train and employ TANF recipients seeking employment opportunities.

3. Strong preference will be given to Offerors experienced in:

Community-based services that promote inclusion of children and adults with Developmental Disabilities

4. Offerors who propose completion of ALL outcomes and activities of a project will be considered responsive and eligible for award, unless a specific project indicates that the project funding may be divided.

A proposal packet may also be requested by writing to the above individual at the Department of Health, Developmental Disabilities Supports Division, 5301 Central Ave Suite 203, Albuquerque NM 87108.

Anyone with a disability who is in need of a reader, amplifier, sign language interpreter or other form of auxiliary aid or service to participate in the conference may contact Travis Goldman at 505-795-1126 or may make contact through the New Mexico Relay Network at 1-800-659-8331.  Public documents including the RFP can be provided in various accessible forms.  Contact Travis Goldman if a summary or other type of accessible form is needed.

Proposals must be received for review at the above office location by 4:00 PM MDT on April 6, 2010. Offerors shall submit one original and three unbound copies of a completed proposal including all required program data and attachments. The original proposal with original signatures should be labeled as "original".

The content of any proposal shall not be disclosed to competing Offerors during the negotiation process. The Procurement Code, Sections 13-1-28 through 13-1-199 NMSA 1978, imposes civil and criminal penalties for its violation.
