The Developmental Disabilities Supports Division (DDSD) Request for Proposals (RFP):

American Recovery and Reinvestment Act (ARRA) Extended Part C Option Fiscal Year 2010 Request for Proposals

The New Mexico Department of Health (DOH), Developmental Disabilities Supports Division (DDSD), is issuing a Request for Proposals (RFP) for the Family Infant Toddler (FIT) Program. The purpose of this procurement is to obtain early intervention services funded under the American Recovery and Reinvestment Act (ARRA) for children in the FIT Program who have turned three and who continue to receive FIT services until they enter preschool special education at the beginning of the following school year. At the end of the State Fiscal Year, an amendment may be negotiated at the discretion of the DOH to reflect funding of services for the next year of the agreement, dependent upon performance, availability of funds and status of DOH priorities.

Successful offerors must agree to comply with all reporting requirements under the American Recovery and Reinvestment Act (ARRA).

A proposal packet may also be requested by writing to the above individual at the Department of Health, Developmental Disabilities Supports Division., 810 San Mateo, Santa Fe, NM 87506 or for more information on the Family Infant Toddler Program see: www.fitprogram.org.

If you are a person with a disability who requires a reader, amplifier, sign language interpreter or any other form of auxiliary aid/service to participate in any aspect of this process, please contact Monique Garza at (505) 476-8830 or by mail at the address listed in the previous paragraph at least ONE WEEKS PRIOR TO APPLICABLE DEADLINE to acquire a copy of the RFP in accessible form. Proposals must be received for review at the above address (if delivering by US Mail) or at Department of Health, Developmental Disabilities Supports Division 810 San Mateo Santa Fe NM 87506 (if sending by UPS, Fed Ex, other courier, or in person) by close of business day on Friday November 13, 2009 Any proposal received after this deadline will be considered non-responsive and not eligible for review.

The content of any proposal shall not be disclosed to competing Offerors during the negotiation process. The Procurement Code, Sections 13-1-28 through 13-1-199 NMSA 1978. Provider agreements shall be awarded to serve New Mexico communities throughout the state. Offerors should specify the geographic area (County(ies) and / or Pueblo / Indian Nation) to be served and should include the amount of funding requested for each area.
