The Developmental Disabilities Supports Division (DDSD) Request for Proposals (RFP):

The New Mexico Department of Health (DOH), Developmental Disabilities Supports Division (DDSD),
is issuing a Request for Proposals (RFP) for Fiscal Year 2012 (July 1, 2011-June 30, 2012).

Proposals from service providers are being sought for Family Infant Toddler Program services in Cibola County;
Respite for children and adults with developmental disabilities and their families in Cibola and San Juan
Counties and Day Habilitation Services to adults in Cibola County. The total amount of funding available
through this RFP is $141,796.00.

The DDSD reserves the right to cancel this RFP and/or to reject any proposal in whole or in part.

The purpose of this procurement is to provide services and supports to individuals with, or at risk
for, developmental disabilities and their families. Individuals and their families have asked for the
services and supports identified within this RFP. Strategies specific to this Performance Measure
are: Provide information and support to individuals, parents, and guardians that supports timely
access to services and promotes self advocacy and self determination.

The proposed provider agreement for fiscal year 2012 shall become effective upon approval of the
Department of Finance and Administration. Extensions are contingent upon sufficient funding, priority
needs of the DOH, and satisfactory performance.

Proposals must be received by close of business day on August 17, 2011. Any proposal received after
this deadline will be considered non-responsive and not eligible for review.
The content of any proposal shall not be disclosed to competing Offerors during the negotiation process.

Offerors shall submit one original and three unbound copies of a completed proposal including all
required program data and attachments. The original proposal with original signatures should be
labeled as "original".

The Procurement Code, Sections 13-1-28 through 13-1-199 NMSA 1978, imposes civil and criminal
penalties for its violation. In addition, the NM Criminal Statutes impose felony penalties for illegal
bribes, gratuities and kickbacks.
