DOH Seal Developmental Disabilities Supports Division, The Central Registry

The Central Registry and how data is collected
The Central Registry is the database that keeps information about each person who applies for services. Services are offered through the Developmental Disability Waiver, State General Funded (SGF) Services, and Intermediate Care Facility for the Mentally Retarded (ICF/MR). Developmental Disability Support Division staff use the information from the Central Registry to determine how many people are requesting services. This helps the Developmental Disabilities Supports Division (DDSD) to plan how best to serve the individual.

The Central Registry is also used for offering services to people who match the definition of mental retardation or related condition and who have a developmental disability. Services are offered in order of the registration date of the individual or family. It is the single point of entry for all Developmental Disabilities Supports Division services except for the FIT and SGF Respite programs. Regional Offices are linked electronically to a centralized database so they can check and track eligibility information and placement of individuals into services.(see Eligibility page).
