Developmental Disability Eligibility Determination:
An individual must have been diagnosed as developmentally disabled according to the following criteria:
A severe chronic disability, other than mental illness that:
1) is attributable to a mental or physical impairment, including the result of trauma to the brain, or combination of mental and physical impairments;
2) is manifested before the person reaches the age of twenty-two years;
3) is expected to continue indefinitely;
4) results in substantial functional limitations in three or more of the following life activity: a. self-care, b. receptive and expressive language, c. learning, d. mobility, e. self-direction, f. capacity for independent living, g. economic self-sufficiency ; and
5) reflects the person's need for a combination and sequence of special, interdisciplinary or generic care treatment or other support and services that are of life-long or extended duration and are individually planned and coordinated.
An individual must have mental retardation or a specific related condition, limited to cerebral palsy, autism (Asperger's Syndrome), seizure disorder, chromosomal disorders, syndrome disorders, inborn errors of metabolism, and developmental disorders of the brain.
Note: Some individuals will not meet the eligibility criteria for services through the Developmental Disabilities Waiver, but may meet the criteria for services through the State General Fund. These services include respite, vocational, day habilitation and residential (these services are limited depending on availability of funding and providers).
