Developmental Disabilities Supports Division provides many services and resources to the independent case management system in New Mexico. This page will lead you to these resources and information about case management for the DD Waiver. If you have questions about case management, please contact the Case Management coordinators in the Regional Offices. We hope you find this page helpful.

DDSD Has two new forms, replacing all related versions of the Case Management Site Visit forms. DDSD is requiring that all Case Management Agencies use these forms only. See Resources for Case Managers below:

DDSD is requiring that all Case Management Agencies use these forms only.
- Monthly Site Visit Form: used for monitoring ISP implementation and progress, services and supports- as well as the health and safety of individuals receiving DD Waiver Services. This is to be used for all Adults, including Young Adults who have graduated or transitioned from school.
- The Children’s Site Visit Form: is to be used to monitor ISP implementation and progress, services and supports, and the health and safety of children receiving DD Waiver services. This is to be used for Children, including Young Adults who have not yet graduated or transitioned from school.
- Included in this site is the Resource Guide for Case Managers or how to do case management within the DD Waiver program, a contact list of regional DDSD case management staff for assistance and many case management tools.
- The Individual Service Plan (ISP) and Instructions. See the new ISP and the process that will assist the individual in developing goals and outcomes that can be easily measured.
- Level of Care Request to Increase Process - Effective 2/01/2010, all requests to increase an individual's level of care must be pre-reviewed by DDSD prior to submission to the utilization review contractor. These tools are to be used to guide case managers in these submissions and to document all activities associated with these DDSD reviews.
