Provider Enrollment
This page contains information on how to become a Developmental Disabilities Supports Division (DDSD) provider of services for the various programs offered to New Mexicans. For existing providers there is summary information for ongoing or renewal preparation requirements. If you have additional questions, please email us.

The central office staff has provided a waiver general checklist for providers interested in preparing in advance for the necessary requirements in order to become a part of the DDSD team. Waiver General Information for Providers is in Adobe pdf (Portable Document Format) format that contains a checklist and other important information about doing business with DDSD.

Secondary Freedom of Choice

This form is used by Case Management Agencies to present to registrants when selecting a Developmental Disabilities (DD) or Medically Fragile (MF) services. It may also be used to create a provider report by a county and a service. To request a Secondary Freedom of Choice form (click here).

Mi Via Waiver Consultant Agency Applications

Annual Performance Contract for Community Access and Adult Habilitation Services

Community Access and Adult Habilitation Services, the Performance Contract Process and Instructions as well as the transmittal letter, Deliverables, and Quarterly Report Format (which is also used for reporting initial baseline information). (more)

Outlier Process and Tools for the Developmentally Disabled (DD) Waiver
Effective 9/1/2010, all requests for Outlier Service must be approved by the DDSD Regional Office. The process and tools contained in this section are to be used for all such requests. If there are questions, please contact your local regional office.

Sample Formats for Documenting Services
DDSD provides sample templates that can be used to document each of the service-types. This is posted on the DDSD website in the Rules tab under DDSD Guidelines. These forms will be updated as necessary to reflect expectations or new formats which may be adapted by providers. See: Technical Assistance Guideline for Providers For Service Delivery Documentation for the templates and more information.

Billing Information
Affiliated Computer Systems (ACS) provides training once every 3 months. One-on-one sessions are provided within the 3-month period. To set up an appointment, please call 1-800-299-7304, extension 190.

The DDSD Monthly Provider Invoice and the Invoice Instructions give providers of Adult State General Fund, Respite and non-direct Family Infant Toddler (FIT) Program services the monthly invoice for and guidance needed to complete the DDSD invoice:

The annual DDSD Statewide Directors meetings began in FY 2006 with the posting of award winners of this meeting to our website.
