Developmental Disabilities Services:
The Developmental Disabilities Waiver Program helps eligible New Mexicans with developmental disabilities maintain and develop skills needed to live a quality life in their local communities. Services can be received from only one Medicaid Waiver Program at a time, even if the individual qualifies for more than one Waiver. There are several waiver programs in addition to the Developmental Disabilities Waiver. State General Funded services are available for those who do not meet medical and financial eligibility requirements for the Developmental Disabilities Waiver Program. Services are available include respite (birth to adult), residential services and vocational habilitation services. Services Available Through the DD Waiver Program:
Services range from Case Management and Day Services to Community Living Supports and Therapies (Physical, Occupational Speech/Language and Behavioral). Other services include Supported Employment, Adult Habilitation, Habilitation Day Care, Nursing, Nutritional Counseling, Personal Care, Respite, Environmental Accessibility adaptations, non-medical transportation and community membership. How to Apply:
Go to your local county office of the Income Support Division. Register by filling out a Medical Assistance Division Waiver Services Registration Form and turn it in to your local county Income Support office. Once you are signed up, keep the income support division informed of changes to your registration information. Your registration date for this program will be the date you turn in the Registration Form to the Income Support Division. Your name will be put on the Central Registry for services. DDSD will send you a letter when services can be considered for you.
Registrations for the Developmental Disabilities Waiver and the Medically Fragile Waiver are now also being taken by the Intake and Eligibility workers at the DDSD Regional Offices. The completed registration form may be mailed or turned in to the Regional Office.
