Medically Fragile Waiver Program:
The Medically Fragile Waiver Program helps New Mexicans with developmental disabilities and a medically fragile condition live in their homes. It is a Medicaid Waiver program that provides the same services that would be given in an institution. Services can be received from only one Medicaid Waiver Program at a time, even if the individual qualifies for more than one Waiver. Services Available Through the Medically Fragile Program:
Services include: Case Management (by Registered Nurses only), Private Duty Nursing (RN & LPN), Home Health Aide, Physical Therapy, Speech Therapy, Occupational Therapy, Psychosocial Counseling, Nutritional Counseling and Respite. Learn more about the Medically Fragile Waiver.

How to Apply:
Go to your local county office of the Income Support Division (check your phone book in the blue section under "State Government - Human Services Department"). Fill out a Waiver Services Registration Form. Once you are signed up, make sure you keep the copy given to you for your records. Also please keep Income Support Division informed if your address changes.

Registrations for the Developmental Disabilities Waiver and the Medically Fragile Waiver are now being taken by the Intake and Eligibility workers at the DDSD Regional Offices.  The completed registration form may be mailed or turned in to the Regional Office.
