Requested changes should be forwarded to the point of contact authority in writing to facilitate a formal change to the New Mexico Department of Health Medical Countermeasure Planning Guide. The point of contact authority for all revisions to this Guide is the Epidemiology and Response Division, Bureau of Health Emergency Management Planning and Resource Management Section, Strategic National Stockpile (SNS) Coordinator. An individual at each regional public health office, division, bureau or agency who is in receipt of this Guide should be responsible for filing any changes they may receive.
A letter providing instructions must accompany any changes. Instructions may include:
1. Adding new pages and destroying old ones;
2. Making minor handwritten changes;
3. Recording requested information relating to the change on this page;
4. Filing a copy of the change at the back of this Guide.
