Case Management

Case Management services are person-centered and support DD Waiver individuals to pursue their desired life outcomes. In general, case management services include:

• Helping with activities and information needed to complete the initial allocation process and annual recertification of Level of Care for DD Waiver;
• Advocacy for individual’s rights, choices and self-determination, as well as promoting individual self-advocacy;
• Assessment;
• Linking to natural and generic supports and services in the individual’s own community,
• Leading person-centered planning and assisting and supporting the individual/family to direct and participate in service planning and decision making;
• Leading Interdisciplinary Team (IDT) meetings to develop or revise an Individual Service Plan ISP;
• Ensuring DD Waiver services are selected through the Secondary Freedom of Choice (SFOC) process;
• Submitting ISPs, budgets and other required information for prior authorization of DD Waiver services;
• Monitoring ISP implementation and progress, services and supports, as well as the health and safety of individuals receiving DD Waiver services; and
• Maintaining and distributing necessary records.
