Congratulations on having a new baby. Whether this is your first, or you have other children in the home who need care and attention, nearly everyone can use a little extra help in the first weeks after delivery. Family Connects New Mexico nurses are trained to answer your questions and if needed, connect you with the support you need.
Services
- A registered nurse visits the home around 3 weeks to check on mom, baby, and the whole family
- Based on need, you may also have up to 2 more visits with a Family Connects New Mexico Nurse, this is either in person or by phone.
- Once you complete your visit(s) Family Connects New Mexico staff will complete a post visit connection call, to ask a few questions about your overall postpartum experience and the services you received.
- When you complete the post visit call you will be offered an incentive as part of our “Just What I Need” promotion.
*Initially available to patients who receive prenatal care at the Eubank Women’s Health Clinic, are residence of Bernalillo County and deliver at UNM Hospital.
